Community-Based Support Services – Intellectual Disabilities
Our Community Habilitation and Day Habilitation programs support adults and children with intellectual disabilities. The supports provided by each program are individualized to the needs and desired outcomes of each person. Activities and opportunities that are built into the individualized planning process are designed to support skill-building; functioning successfully and more independently in the community; and developing meaningful relationships and community connections. Individuals enjoy being an active and productive member of their communities.
Community Habilitation
Madison and Onondaga Counties
Community Habilitation provides person-centered, one-to-one support for children and adults with intellectual disabilities at home and in the community. The individual and family can expect not only training and assistance in areas of need but also access to the community for desired activities. Outings provide an opportunity to work on skills in the community. Activities may include visits to local points of interest, attendance at cultural performances, opportunities for exercise and volunteer work, and enjoying time spent with friends.

Home and Community-Based Supports
Madison and Onondaga Counties
We provide support to children in settings that enable them to remain at home and in the community. These children would otherwise be at risk of admission to institutional levels of care. We believe in a family-driven, youth-guided approach that values the strengths, preferences, needs, and desired outcome of the individual. We provide an array of services—focused on success at home, in school, and in other natural environments—that evolve over time to meet the changing needs of children.
