COMPREHENSIVE CARE COORDINATION
Care Coordination is provided through the NYS Health Homes program and is specifically designed to help children, families and adults connect with an array of services to address all of their needs; physical health, behavioral health, education and social supports. Individuals and families will have one point of contact, their Care Manager, who provides guidance and support with enrolling and accessing services to stay healthy, succeed at school and remain involved in their communities.

Adults, Children, & Families
Liberty Resources’ Care Coordination provides tailored and individualized supports to meet the needs of children, families and individuals. Our Care Managers create open lines of communications between the different service providers to avoid unnecessary duplication and improve the health and well-being of those we serve. Care Coordination is dedicated to the health and well-being of the whole person. We strive to connect families and individuals with the services they need and want – in order to be happier, and healthier.

ELIGIBILITY INFORMATION
Individuals must be enrolled in Medicaid and meet eligibility criteria by having two or more chronic health conditions or one of the following single qualifying conditions: HIV/AIDS, Serious Emotional Disturbance, Complex Trauma, Sickle Cell Disease and Serious Mental Illness (SMI) in adults. Liberty Resources is a certified provider in Madison County, which allows individuals residing in this county to be enrolled who are not eligible for Medicaid.

Vision & Hearing Loss
Aurora of CNY, an affiliate and close partner of Liberty Resources, provides Care Coordination services for children, adults and seniors with vision or hearing loss. The Care Coordination services offered by Aurora of CNY provide access to and coordination of healthcare providers and community supports so that individuals can meet their goals of living a productive, healthy and independent life. Coordinating with providers for special issues relating to vision and/or hearing loss needs is a specialty of their care and support.
Early Childhood
Liberty POST, a division of Liberty Resources, has extensive experience with providing children and families with early childhood diagnostic, therapeutic and special education services. The Care Coordination services of Liberty POST helps families navigate the complicated regulatory system of school districts, health care, and Medicaid.
