Geri Johnson joined Liberty Resources’ Substance Use Disorder team as a Therapist in the Integrated Health Care Clinic. She is a Masters level CASAC and has worked in the Substance Use field for over 20 years. Geri also holds a Master of Divinity from Naropa University and completed a chaplain residency in a Level 1 trauma hospital in York, PA. She is licensed in North Carolina as a Clinical Mental Health Counselor and is a Certified Clinical Trauma Professional. She is also a mother of two adult daughters, and a grandmother to seven. Geri has a mastery at baking cookies, tent camping, and spending time alone in the wilderness.

Alann Weissman-Ward is a Board Certified Addiction Medicine and Family Medicine Physician. Prior to his role at Liberty Resources, he served as a Medical Director for Commonwealth, and inpatient rehabilitation program that is part of Crouse Hospital Chemical Dependency Treatment Services. Dr. Weissman-Ward has provided care for socially and economically overburdened patients in a multitude of settings, including Urban Indian Health Centers, County Jails, and countrywide via telemedicine. He continues working for Plume, a telehealth company that provides Hormone Replacement Therapy for non-binary/trans/gender expansive patients and is a strong advocate and provider of gender affirming healthcare.

Tom has been working in the field since 2009 and has provided direct therapeutic services and clinical supervision at a variety of Central New York addiction recovery providers, including Crouse Hospital, and Upstate University Hospital. He also served as a Medical Education Facilitator at St. Joseph’s Family Medicine Residency program, focusing on mental health and substance use treatment education. Tom is a native of Central New York; loves skiing with his family in Vermont and continues to work for Habit for Humanity in the U.S. and abroad.

Whitney started with Liberty Resources in 2019 as a mental health RN. She resides in Syracuse, NY with her fiancé, stepson, dog and kitten. She is a strong advocate for patients and believes that everyone can lead a healthy life no matter what obstacles are present. Whitney graduated from JCC, worked in the ICU for three years and then moved to the outpatient role with the desire to help an underserved population. She started working with the substance use disorder division due to her passion for assisting people struggling with addiction and her strong belief that recovery is possible for everyone.

We will be there as soon as possible. At the hospital, a nurse will offer to call the police for you. This is your choice. The police will not be called automatically, either by the hospital or by Center staff. This is to protect your privacy. There are some circumstances in which hospital staff are required to notify the police: if a gun or knife was used or if there is reason to suspect child sexual abuse. How much or little you choose to tell the police in these cases is still your choice; the intent is to protect you or someone else from further harm in these grave situations.

Melissa Jillson is a Licensed Master Social Worker and has been working in the field of human services since 2004. Most recently Melissa assumed the role of Vice President of Clinic and Community Based services and oversees the operation and development of the Integrated Healthcare Clinic, Adult and Children Health Home Care Management, and Adult Home and Community Based Services. Melissa received her Bachelor of Science in Psychology from SUNY Cortland and her Master’s in Social Work from Syracuse University. She has successfully designed and executed the start-up up several programs vital to the community including community-based residential, crisis stabilization and peer based services, for individuals with complex behavioral health issues. Melissa is well versed in budget planning and oversight, contract management, and in engaging external stakeholders to support new community based initiatives.

After graduating with his RN from Crouse College of Nursing, Clark began his career at Oswego Health, leaving there as a Senior Registered Nurse to take on more responsibility as a Nurse Manager with Crouse Hospital. Clark is currently working towards his MBA in Healthcare Administration and brings extensive leadership experience as well as an innate passion and enthusiasm for excellent patient care to our Primary Care team.

Jody Coppola started her career in Nursing in the Intensive Care Unit more than 30 years ago. She became an educator at Community General Hospital in 1999 while completing her master’s degree in the Family Nurse Practitioner Program at Upstate Medical University. Jody provided primary care in her own family run practice until 2014, and she concurrently taught as an Assistant Clinical Professor at Upstate Medical University until 2017. Jody then took a position teaching as a Professor of Practice at LeMoyne College in the Family Nurse Practitioner program, and she went on to receive her Doctorate of Nursing Practice at the University of Buffalo in 2018. Today she practices as a Family Nurse Practitioner at Liberty Resources part time while she continues to teach at LeMoyne College full time. As a provider at Liberty Resources, she knows that Liberty Resources is leading the way in integrated care which evidence shows improves patient outcomes and reduces health care costs.

Kelly Pawlikowski has been a valued employee for over 20 years, starting her career as a Direct Support Professional in our Developmental Disability Services division. Kelly transferred to Liberty’s Venture House program in 2002 – a mental health transitional residential program in Oneida, NY where she was ultimately promoted to Program Supervisor until she obtained her Bachelor of Science in Nursing in 2015 and began working at Liberty’s Integrated Health Center in Syracuse. In 2020 Kelly was promoted to Nursing Coordinator for Primary Care and Behavioral Health – a vital leadership role for the Integrated Health Center. Kelly’s energy, experience and agency knowledge are invaluable assets for the Liberty’s Primary Care team.

Laura has been working in Special Education for more than 27 years and in Early Intervention and Preschool for 22 of those years. Laura became the vice president of Liberty POST in January 2019. Before this, Laura was the Syracuse regional program director for 10 years and became the director of school administration in 2014. Laura holds a bachelor’s in early-childhood development, a master of special education, and a CAS in Educational Leadership/School Administration. She is also an adjunct instructor in the School of Education at Syracuse University. Patients, staff, and any individual entering our facility are pre-screened for virus symptoms and have their temperature checked upon arrival with touch-free thermometers.

Ample room is provided within our waiting room and front entrance to ensure that patients can remain six feet from each other at all times.

Scheduled appointments will be staggered to limit the number of patients on-site at one time.

MST is designed for youth with complex clinical, social, and educational problems. Delivered in homes, neighborhoods, schools, and communities by Master’s level professionals, MST addresses the multiple factors known to be related to delinquency. The program goal is to prevent placement and maintain the youth in the home and community.

Initial assessments, including gathering data from parents, children, caseworkers, probation officers, and other relevant sources.
Collaboration with family members, referral source staff, and Probation staff in providing treatment to youth and family, sharing progress, and reaching treatment goals.
Development of preliminary and comprehensive weekly treatment plans.
Home-based, therapeutic counseling to families, including individual and family sessions.
Crisis intervention services.

Eligibility Criteria

MST Services are available to youth ages 12-17 that are at risk of out-of-home placement and involved in the juvenile-justice system.

Placement Diversion & Family Reunification Program

Our Mission

The mission of the program is to reunite children with their families and provide the services necessary to decrease continued juvenile offenses, prevent further placement outside the home, and support the family to maintain long-term stability.

PD/FRP is an outcome data-driven program with its foundation in the evidence-based Structural Family Therapy Model. The Placement Diversion Program is designed for youth who are assessed to be at risk of out-of-home placement due to emotional, behavioral, and familial problems. The Family Reunification Program supports the transition of youth from placement into the family, school, and community. Both programs aim to increase youth success by focusing on family functioning and positive parenting behaviors.

Liberty Resources offers Telehealth services, live-interactive videoconferencing with highly trained clinicians, through our Integrated Health Care and our Texas Family Services programs. Telehealth Counseling and Telehealth Psychiatry involve direct interaction between a counselor or psychiatrist and the client.

Telehealth helps meet patient’s needs for convenient, affordable, and readily accessible mental health services.

Residents in the Apartment Treatment Program have the opportunity to enhance their independent living skills in a less restrictive setting and under realistic conditions of personal responsibility, while still having access to program staff on a daily basis for instruction, assistance, and support.

Our Mission

Supportive Living for Substance-Use-Disorder Recovery

This program is the next step in an individual’s road to recovery. We support clients with their housing and treatments to create stability and safety. This program consists of scattered apartment sites and housing. All the housing is conveniently located and within walking distance of desired community resources.

Staff are available to residents 24 hours per day, 7 days per week.

Participants of this program will be 18 years of age or older with a diagnosis of alcohol/chemical dependency. Participants will complete a course in chemical-dependence treatment or rehabilitation in an inpatient or outpatient setting and have an established support system and recovery plan. Participants must be able to function without 24-hour-a-day on-site supervision.

For a Loved One / Family Support

The Family Support Navigator (FSN) assists families with accessing local treatment and support service.
The FSN will develop relationships with Monroe County addiction prevention, treatment and recovery services, managed-care organizations, local substance-use-disorder councils, and community stakeholders too.

For Myself / Peer Advocate

The Peer Advocate helps establish connections to addiction treatment after an opioid-overdose reversal, after discharge from another addiction-related ED visit, or as needed in other settings.

Emergency Response to Opioid Overdose

The Liberty Resources Recovery Support Navigator Team (RSN) has teamed up with the Monroe County Department of Public Health (DPH) and created a collaborative pilot program. The goal of this program is to connect Liberty Resources’ Peer Specialists and Family Support Specialists to individuals and families who have recently experienced an overdose.

The DPH receives information on anyone who has suffered an overdose, from both the Rochester Police and Sheriff’s departments within 24 hours of that overdose. The DPH then informs the Recovery Support Navigator Team, and we deploy a Peer and/or Family Support Specialist to that individual and/or family upon receiving the referral. The team starts with telephonic outreach and then works together with that individual in a person-centered approach to develop a plan of support. The RSN Team takes a Peer Model/Harm Reduction approach as well, which means that we are not here to tell anyone what to do, but rather to deeply listen to the needs of the individual and/or family.

With the support of your parent/advocate, Broker, Care Manager, and Circle of Support, you will have the freedom to manage your own budget and staff with help from a Fiscal Intermediary and Support Broker (Self-Direction with Budget and Employer Authority) or we can help you self-direct if you prefer. (agency-supported self-direction).
In the process of developing your self-directed plan and budget, you will work with a Support Broker (often the same as the Start-Up Broker) who helps you manage your plan and coordinates the quarterly Circle of Support meetings.
Supports and services can be provided by either staff that are hired directly by you or through agency-supported staffing.

Self-Directed Services are based on increasing the options and choices you have for full inclusion and access to your community, greater independence in your life, and the ability to exercise freedom in the types of services and supports you receive.
Integration and access to the community means that you will have chances to find competitive employment, having access to housing options in the neighborhoods where you want to live, and participating in local activities and accessing services for people with or without disabilities.

The HUD Permanent Supportive Housing (PSH) program assists persons with substance-use disorders and mental health issues that create barriers to permanent housing. The HUD PSH program is composed of 29 units spread throughout Onondaga County. Each tenant has a private apartment and is responsible for an appropriate portion of the rent every month.

Our Mission

Mental health issues can impact many facets of daily living including access to permanent housing, a crucial component of maintaining an individual’s health. Our Mental Health Supportive Housing programs provide services and financial supports for individuals transitioning from a residential setting and also for individuals who are at risk of homelessness.

Housing Placement

Our Housing Counselors ensure that individuals have the ability to exercise their right to choose where they are going to live, taking into consideration the recipient’s functional skills, the range of affordable housing options available in the area, and the type and extent of services and resources that recipients require to maintain their residence within the community.

Financial Supports

The program provides financial assistance to recipients who meet income guidelines. Liberty Resources successfully provides rental stipends, moving costs, security deposits, and utility assistance to more than 80 recipients a year.

Case Management Services

Housing Counselors offer case management services, supportive counseling—including crisis intervention, ongoing assessment and support planning, and independent-living skills training. We encourage participants to develop natural community supports and to use community resources to strengthen their bond to their neighborhood and encourage integration into community.

Each program provides services to adults, age 18 or older, with a documented mental health disorder. Substance-use disorders often create housing barriers for individuals. Finding and maintaining safe housing is a crucial component in recovery. Our permanent-housing programs provide services that are appropriate to the needs of each individual we serve, with the goal of greater independence.

Supportive services are developed to meet the needs of each individual we serve. This is established through individual assessment and the development of an individualized service plan. Program staff, either directly or through linkages, assist tenants with accessing resources that will enhance their ability to secure employment, including educational opportunities, job-readiness skills, vocational training, and employment placement and retention. Apartments are turnkey, and all tenants have the option to take over responsibility of the lease when it is determined that our services are no longer needed.

Video-based tele counseling helps meet patients’ needs for convenient, affordable, and readily accessible mental health services. These tele counseling services are provided by licensed mental health professionals, via computer-based audio-visual software, in the Central Texas Region. Our Mission

Liberty Resources’ case management services help families to cope with and resolve the environmental, social, emotional, and economic issues that may be impacting their ability to provide and maintain a healthy family environment.

Customized Care for Families

These services help families reduce the risks of placement, minimize disruption within the home, and increase the parent/caretaker’s skills and support system.
A plan is developed around the family’s strengths and current support systems. Goals are monitored to prevent placement and to minimize the risk of disruption.
Liberty Resources’ offers of support in the family’s home and in the community include support for all members of the family.
We coordinate services with other providers—to provide support to families, as needed; and to monitor compliance with any court orders and formal recommendations from Child Protective Services.
Skill-building services can be conducted in addition to case-management services.

Referrals

Families are referred to the program by county or community providers. The majority of families who have been referred to the program have an indicated child protective report, which signals the need for preventive services in order to increase safety and stability in the home.

Family Reunification & Support Therapy

Our Mission

Family Reunification and Support Therapy provides comprehensive home-based therapy to families whose children had been placed in a higher level of care. The purpose of the program is to reunite these children with their families and provide the necessary services that will support families in preventing further placement of their children and in maintaining long-term stability.

Customized Care for Families

Liberty Resources therapists offer family-therapy services, based on the family’s goal for reunification.
This program utilizes solution-focused and structural family therapy in its treatment approach.
It is our belief that home is the best place for change to occur.
We provide services to help families in the transition from feelings of doubt and hopelessness to feelings of competence and possibility in the child’s return home.
This program provides opportunities for families to experiment with how to do things differently.
We also support the family with case management aimed at the coordination of service providers and the elimination of the need for a social services caseworker.
Families have access to 24-hour crisis services, to access support whenever it might be needed.

Children who have not yet reached their 18th birthday and who have been adjudicated as a PINS or juvenile delinquent and who are in the custody of the Madison County Department of Social Services and who have been placed in an institutional level of care are eligible for these services. The family will also demonstrate an ability to benefit from services through their level of involvement in their child’s current treatment; and if the child is at risk of further placement either due to a recurrence of the previous circumstances surrounding his/her placement or due to not achieving goals set forth in their comprehensive treatment plan.

Our Mission

The program’s primary goal is to decrease a youth’s involvement with the juvenile-justice system by addressing behaviors that may range from truancy, substance-use disorders, criminal activity, or other common antisocial behaviors. MST accomplishes this task by looking at all of the different systems in a youth’s life (individual, family, school, peer, and community) and then working with the youth’s caregivers to develop interventions that will help reduce any negative behaviors.

Internationally Recognized Evidence-Based Practice

MST is an internationally known program that was first conceived in the 1970s at the Medical University of South Carolina.
MST research indicated the need for treating adolescents in their homes and for addressing all of the systems that impacted their lives on a daily basis.
After many years of research, word of MST’s success began to spread, and this led to the creation of MST Services in 1996.
MST Services is a licensed organization that offers assistance in developing and maintaining MST program services worldwide.
Liberty Resources has been licensed as an MST Network Provider for more than 10 years. This licensure allows us to provide MST services, to provide MST clinical oversight, and to develop the MST model internationally.

Scope of Services

MST’s interventions aim to improve caregiver parenting skills by developing discipline strategies, strengthening positive relationships among family members, increasing prosocial activities for youth, improving youth’s performance in school, and developing a strong network of supports for the youth and family to help caregivers maintain positive changes.
To reach these goals, the family meets with a therapist multiple times per week to participate in setting goals and developing interventions for the youth.
MST uses a strengths-based model to build upon the positive skills that the family is already utilizing.
Family members get to build their own goals and to work with their therapist to reach the outcomes that are most important to them.
At the end of the treatment period, success is measured by eliminating the need for out-of-home placement; reducing contact with police and other members of the juvenile-justice system; and improving behavior in the home, school, and community.

Substance-Use Specialization

The MST program provides substance-use treatment through the MST model.
This unique treatment program allows youth who are struggling with substance-use issues to work together with their MST therapist and their caregiver to identify their personal triggers and to develop self-management plans to reduce their risk of drug or alcohol use.
Through this program, youth are able to create a personalized plan that targets their struggles with substances, and caregivers are given the opportunity to learn more about their youth’s drug use and to become an active part of the support system that helps reduce this behavior.

Qualified Professionals

All staff have clinical experience and hold a master’s degree in social work; marriage-and-family therapy; counseling; or a related discipline that adheres to the Multisystemic Therapy Model, which is grounded in research and evidence that best supports success for every family.

In-school therapists are able to meet students during the school day, in the school setting.

The Liberty Resources Behavioral Health Center team is composed of licensed therapists (social work, marriage and family therapy, mental health counseling, addiction specialists); psychiatrists; psychiatric nurse practitioners; family practice nurse practitioners, and a skilled team of customer service staff.
We are a training center of excellence for students at Syracuse University, Upstate Medical Center, and other local institutions.
Our multidisciplinary approach ensures that treatment services are integrated and coordinated with primary care physicians. Our holistic approach to treatment incorporates wellness and the notion that physical health is impacted by emotional health and vice versa.

Mesa Commons Permanent Housing

Our Mission

Liberty Resources provides permanent housing to families who are homeless or housing vulnerable through our MESA Commons site, comprising two- and three-bedroom townhouse units.

Amenities

Each tenant has a private entry through a community courtyard.
All units have a half-bath upstairs and a full bathroom downstairs.
Included in the rent are heat, electricity, hot water, cooking fuel, stove, refrigerator, and garbage collection.
All units also include air conditioning a washing machine, and a dryer.
All living rooms and bedrooms are cable-ready and have telephone jacks.
No security deposit is required at MESA Commons.
Leases are required—with the period of the lease being month to month.

Supports

Sober and drug-free housing environment, with linkages to treatment and 12-Step meetings.
Case management, tenant conflict resolution, and linkages and referrals to Syracuse area services.
A variety of services are provided to residents at MESA, including monthly grocery assistance, tenant meetings, and on-site counseling.

Eligibility criteria

Applicants who are at least 18 years of age with a documented HIV/AIDS diagnosis who are homeless or have inadequate housing with income below the federal poverty level.

Transitional Housing – DePalmer House

Onondaga County

Our Mission

Liberty Resources offers transitional housing for homeless persons at our DePalmer location. The primary goal of this program is to provide an environment and level of services that will enable residents to attain and maintain safe, affordable permanent housing of their choice.

At least 92% of the clients who are served at DePalmer House are individuals in recovery from chemical addiction. At least two-thirds of the consumers who are served possess a diagnosed—and often untreated—coexisting psychiatric disorder. Many have had previous, recurrent episodes of homelessness.

DePalmer House is centrally located on the west side of Syracuse and is maintained as a confidential site. The two-story house consists of 10 beds with a full kitchen and dining room, two living rooms, three and a half baths, and a full basement complete with storage. Also, there are two staff offices, which serve as confidential counseling areas. The program is staffed 24 hours a day by a program supervisor, assistant supervisor, three direct support professionals (including an awake overnight counselor), and four part-time relief staff.

Scope of Services

Supportive services offered within the program have allowed residents to address longstanding addiction issues that impede their ability to meet their housing needs. Other services-assist residents with complying with their medical treatment regimen, allowing more-fragile residents to improve their health to the point of being able to move into independent housing. DePalmer House program staff provide a variety of support services to meet the special needs of the population served.

DePalmer House offers linkages with social services, nutrition programs, local case management programs, chemical dependency treatment and mental health programs, health care, transportation, and other services and resources.

At the DePalmer House, patients’ goals include self-sufficiency and the attainment of permanent housing. The program staff assess each individual for needed services and assist them in connecting with these services.

Eligibility criteria

Individuals 18 years or older with a documented HIV/AIDS diagnosis and in a homeless or inadequate housing status who fall below the federal poverty level are eligible for this program. Referrals are routinely accepted from a variety of sources including hospitals, case management agencies, correctional facilities, drug and alcohol treatment centers, and area shelters. Self-referrals are also accepted. Applicants are required to attest, in writing, to their willingness to participate actively in their service plan and to abide by program rules.

Kinship Care

The Liberty Resources Kinship Caregiver program provides services to families in Oswego County who are providing full-time care on a formal or informal basis for youth with whom they have a connection by blood or prior relationship. We provide a level of care and support that will help the nonparent caregivers of Oswego County to provide a safe and nurturing environment in which youth can thrive.

The Independent Living (IL) Skills program is a comprehensive, educational program serving Onondaga County youth in foster care ages 14–21. Program components include Group Skills Instruction; Individualized Assessments; Discussion Group; Annual Appreciation Event; Volunteer Experience; Recreation Activities; and participation at the annual NYS Youth Speak-Out & Event.

Designed for Youth

Helps youth in foster care learn the necessary information and develop the valuable skills to prepare them for living independently after foster care.
Provides learning everyday tasks, such as budgeting and cleaning, through fun and interesting instruction.
Designed to build assets proven to create youth success, among them: Support, Empowerment, Connection, and Positive Use of Time.

Activities to Support & Grow

Discussion Groups focus on the common issues and feelings experienced by adolescents in foster care.
An Appreciation Event is designed to allow youth to select those individuals in their life (ex. family, friends, caseworkers, and/or foster parents) that they would like to treat to a dinner and thank for their support.
Volunteer Experience provides youth with an empowering experience that comes from helping others, while also exposing them to career opportunities and challenging their skills in the process.
Recreation Activities provide youth with an opportunity to make constructive use of their leisure time and promote a positive connection with the youth’s community.
Youth Speak-Out annual event allows NYS foster youth to come together for a day of workshops, networking, and advocacy.

Our Mission

The Family Foster Care Program is a foster boarding-home program whose staff and foster parents are highly skilled and specialized in providing therapeutic intervention, case management and care coordination, skill building, and independent living for children and youth who have experienced trauma as it relates to being removed from the home due to abuse and/or neglect.

This program is designed to provide clinical and case management support to children, their biological families, and the foster families who participate in the program. The program goal is to provide the level of care and support that would enable children to be successful in a community-based setting. The primary purpose of the program is the prevention of institutionalization for these children.

Therapeutic Counseling & Care Coordination

Master’s level therapists offer home-based, individual and family counseling to children and their families, as well as intensive support to the foster families.
Care Coordination and case management services are provided in an effort to coordinate all services.

Family and Youth Are Supported

Youth Specialists access local resources to provide treatment-specific recreational activities and skill-building for the children and to provide respite for the foster parents.
A foster parent training group meets monthly.
Group activities—such as picnics, a Thanksgiving dinner, and a foster-parent banquet
Attendance for interested youth at the State-wide Youth Speak-Out

Preparing New Foster Families

In addition to providing services to children, Liberty Resources recruits and certifies the foster families with whom these children are placed.

Typically, referrals are made by the County for those youths that are close to being placed at a residential or critical care facility (RTC or RTF) or those who have been discharged from a residential facility and may be transitioning back into the community.

Our Mission

Liberty Resources Supports for Employment is committed to helping individuals find and maintain long term employment. Through meaningful work, the individuals we serve can find purpose, fulfillment and a true connection to the communities in which they live.

Finding Employment

Each individual is assigned a team of employment specialists.
The team works together through screening and assessment, job development, intensive training, and extended services once established in a job.
We work closely to understand the person’s job interests, preferences skills, and experience.
We develop relationships with local employers to help provide great opportunities for those we serve.

On-the-Job Coaching and Support

During the intensive training period, the participant learns the job with the help of a job coach.
Extended services provide continued support that helps the employee and employer maintain a successful working relationship.
A successful outcome occurs each time a person secures employment, trains for that employment with or without supports, and is able to maintain that position for at least 90 days after intense supports have been reduced.

Eligibility criteria

Candidates in this program will meet, at minimum, OPWDD, OMH, ACCES-VR, and CBVH criteria for having a significant disability and must have a vocational goal that focuses on competitive community employment.

Supervised Community Living

Our Mission

Our supervised residential facilities provide 24-hour supervision to adults with intellectual disabilities. The residential facilities are focused on each individual’s needs and community inclusion. Our caring Direct Support Professionals are trained in CPR/first aid, medication administration procedures, and crisis intervention techniques. Registered Nurses provide essential oversight and resources at each home—ensuring the highest quality of medical care.

Health and Wellness

Our staff promote the health and wellness of all individuals through health maintenance and prevention strategies, accurate medication administration, knowledge of general health and safety rules, and first-aid and emergency procedures. Direct-support professionals help individuals complete personal-care activities while ensuring an individual’s privacy and promoting independence.

Individual Empowerment

We cultivate and promote the ability of the individual to lead a self-determining life by providing the support and information that are necessary to build self-esteem, assertiveness, and independence. Direct-support professionals show respect for individuals by soliciting and honoring choices and by encouraging personal responsibility and independence in all day-to-day events and activities.

Household Management

Direct-support professionals assist the individuals who are responsible for household management (e.g., shopping, meal preparation, laundry, cleaning, and decorating) and transportation, with the goal of maximizing the individual’s skills, abilities, and independence.

Advocacy

We are knowledgeable about the diverse challenges facing individuals, and we understand our role as an advocate. Liberty Resources Community-Based Support Services are developed through an initial assessment and a close collaboration between our team and the individuals we serve. Our goal is to provide and connect people with the supports and resources they need to be happy, healthy and continue with their mental health and/or substance use recovery.

Home and Community-Based Supports

Onondaga County

Our Mission

Liberty Resources provides support to children in settings that enable them to remain at home and in the community. These children would otherwise be at risk of admission to institutional levels of care. Liberty supports a family-driven, youth-guided approach that values the strengths, preferences, needs, and desired outcomes of the individual. We provide an array of services—focused on success at home, in school, and in other natural environments—that evolve over time to meet the changing needs of children.

Community Habilitation

Assistance with acquisition, maintenance, and enhancement of skills necessary to perform Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), and/or Health-Related Tasks delivered in the community (noncertified) settings

Caregiver/Family Supports And Services

Services enhance the child/youth’s ability—regardless of disability (developmental, physical, and/or behavioral)—to function as part of a caregiver/family unit. And they also enhance the caregiver/family’s ability to care for the child/youth in the home and/or community.
“Family” is broadly defined: It can include families created through birth, foster care, adoption, or a self-created unit.

Respite

This service focuses on short-term assistance provided to children/youth, regardless of disability (developmental, physical, and/or behavioral), due to the absence of, or need for, relief of the child or the child’s family caregiver.
Services can be provided in a planned mode or can be delivered in a crisis situation.

Employment Services

Prevocational Services are individually designed to prepare a youth (age 14 or older) to engage in paid work, volunteer work, or career exploration.
Supported employment prepares youth with disabilities (age 14 or older) to engage in paid work. Supported-employment services provide assistance to participants with disabilities as they perform in a work setting.

Community Self-advocacy Training And Supports

Provide family, caregivers, and collateral contacts with techniques and information that are generally not available—so that they can better respond to the needs of the participant. Community Self-Advocacy Training and Support is intended to help the child/youth, family/caregiver, and collateral contacts understand and address the participant’s needs related to their disability(ies).

Family Peer-supports Services

An array of formal and informal activities and supports that are provided to families who are caring for / raising a child who is experiencing social, emotional, medical, developmental, substance-use, and/or behavioral challenges in their home, school, placement, and/or community.
Provides a structured, strength-based relationship between a Family Peer Advocate (FPA) and the parent/family member/caregiver for the benefit of the child/youth.

Crisis Intervention

Crisis Intervention (CI) services are mobile services provided to children/youth under age 21 who are identified as experiencing an acute psychological/emotional change that results in a marked increase in personal distress and which exceeds the abilities and the resources of those involved (e.g., family, provider, community member) to effectively resolve it.

Eligibility criteria

This service is available to any Medicaid-eligible child/youth under the age of 21 who, upon assessment, is deemed to be appropriate for the services.

Our Mission

Liberty Resources supports Medicaid-eligible children & youth who have mental health needs by providing them with clinical services that can be delivered in the community. Depending upon the unique needs of the child, services can be provided/accessed individually or in a coordinated comprehensive manner.

Therapy Services

Provide assessments for mental health and/or substance-use needs
Identify strengths and abilities through individual and group therapies
Get individual, group, or family therapy where you are most comfortable and where needs are best met; this can be at home and/or in other places in the community

Rehabilitation Services

Learn to incorporate therapy goals into everyday life, and receive extra support to manage medication

Family Peer-Support Services

Get support if you are raising youth with mental health and/or substance-use challenges

Steve Russell

Steve has been working at Liberty Resources in the Developmental Disabilities division, since 1997. After receiving an undergraduate degree in anthropology from SUNY Potsdam, he found his way to Liberty Resources, starting as a residence counselor in a group home in Munnsville, New York. This experience, and development work in day services and housing, prepared him for his current role as the vice president of the division. Steve has a master of arts in public administration from the Maxwell School of Citizenship and Public Affairs, at Syracuse University. In his spare time, he enjoys working with the Fayetteville Manlius Little League, mountain biking, and any time outdoors. Steve resides in Fayetteville, New York, with his wife and three children.

David Harris

David’s original affiliation with Liberty Resources was as a member of the board of directors, in 1996. After 32 years of experience working in healthcare administration and higher education, he resigned from Liberty’s board and became a member of Liberty’s leadership team and a corporate compliance officer. David oversees IT, Business Intelligence, Quality Assurance, and Human Resources. David holds a master of urban planning from SUNY Buffalo School of Architecture. More recently, he completed a master of public administration with a Certificate of Advanced Study in Health Systems Management and Policy from the Maxwell School of Citizenship and Public Affairs at Syracuse University.

Marta Durkin

Marta is a Licensed Master Social Worker with more than 20 years of experience in developing a variety of specialty services to individuals with complex behavioral health issues. She received her bachelor of science in social work from Syracuse University and received her master of social work from Columbia University. Marta came to Liberty Resources in 2008 as a director and assumed her role as vice president of Behavioral Healthcare in 2010. She oversees the operations and development of an array of peer-driven, community-based, and residential services. Before joining Liberty, Marta spent a decade managing a variety of behavioral health clinics and prevention programs throughout New York City.

Kelli began her career at Liberty Resources in September 2018. As a member of Liberty’s executive team, she has a strategic role in the overall management of the company: Kelli oversees the planning, implementation, leadership, and control of all financial-related activities.

Kelli has more than 19 years of experience in the accounting and healthcare-finance industry. Before that, she served as chief financial officer at Crouse Health, in Syracuse. Kelli holds a bachelor of business administration and accounting from Niagara University and holds a master of business administration from the Whitman School of Management, at Syracuse University. Kelli resides in Cicero with her husband and their three children.

Carl Coyle, MSW

Carl M. Coyle, MSW, is CEO of Liberty Resources, Inc., and has led the organization since 1989. He leads more than 1,400 professional staff and 85 public health programs—serving more than 18,000 individuals annually. Under Carl’s leadership, the organization has expanded significantly since its beginning in 1979, with a current operating budget of nearly $80M. He has devoted his professional life to meeting the needs of the most vulnerable of our society. Regionally and nationally, Carl is sought after not only for his depth of experience in public-health delivery and management but also for his insight into the underlying issues. Throughout his tenure at Liberty Resources, Carl has been an ardent proponent of meeting community needs and of diversifying Liberty Resources’ service mix to an ever-expanding geographic footprint. He has also taught internationally on entrepreneurship and business management and has held a variety of instructor positions throughout his career. He resides in Fayetteville, New York, with his wife. Carl has two children and four grandchildren.

Kortney Dale

Kortney began employment with Liberty Resources as an MST therapist, in 2006. Her passion for providing quality services to families, along with developing and cultivating leadership skills in employees, paved the way for several leadership roles, leading to her current role of Family Services vice president. Kortney earned a bachelor of arts in psychology and a master of social work from Syracuse University, where she stays connected to the School of Social Work as a visiting seminar instructor/facilitator. Kortney also serves on the board of trustees at Contact Community Services. Kortney currently resides in Charlotte, North Carolina, with her husband and children.

Jocelyn Coleman

Physical Therapist

Jocelyn began working at Liberty POST in 2017, after graduating with a doctorate in physical therapy from New York Medical College. Her experience before POST included working with children and adults of all ages in outpatient, school, and hospital settings. Working at POST, she treats children ages newborn to 5 years who have a variety of diagnoses, including Down syndrome, autism, prematurity, cerebral palsy, torticollis, and general motor delays. Her interests and areas of continuing education include motor planning, NDT-handling techniques, primitive reflexes, adaptive equipment, and sensory integration.

Our Features

Social adult day services provide care to frail and disabled older adults in a supervised group setting within the community. All services are based on an individual care plan and on assessment of the participant’s needs and goals. Social adult day services provide a break to caregivers, and gives peace of mind to working caregivers.

Our Features

Our program is specifically designed to meet the needs of individuals requiring cognitive, physical, emotional, and behavioral rehabilitation. All of our services are focused on the needs and desired outcomes of the individuals and families we serve.

Service Coordination

The Service Coordinator helps the individual to attain services and to apply to the HCBS/TBI waiver. They are responsible for coordinating and monitoring the provision of all services in the individual’s Service Plan. Together, the Service Coordinator and the individual collaborate to develop the individualized Service Plan, which reflects the individual’s goals, interests, and desired outcomes. Also, the Service Coordinator is responsible for continuing the development, implementation, monitoring, and provision of the Service Plan on an ongoing basis. The Service Coordinator serves as an advocate for the individual in ensuring that all services are implemented as outlined in the Service Plan.

Case Management

In conjunction with residential services, Liberty Resources also offers Case Management. A Case Manager is designated to help recipients of extended services to receive the most-appropriate services to meet their needs. The Case Manager and the individual collaborate to develop an individualized plan of care, which reflects the individual’s goals, interests, and desired outcomes. The Case Manager also ensures that the provisions of services meet the policy requirements and standards that have been set by the insurance carrier. Also, the Case Manager is responsible for continuing the development, implementation, monitoring, and provision of the patient’s individualized plan on an ongoing basis. The Case Manager not only serves as an advocate for the individual in ensuring that all services are implemented as outlined in the plan but also coordinates and collaborates with any internal or external agencies or providers that are needed to effectively facilitate the plan.

Independent-Living Skills Training

Independent-Living Skills Training and Development services are designed to improve the ability of the individual to live as independently as possible in the community. ILST services may be provided in the individual’s home or in the community. ILST services may include assessment; training; and supervision of, or assistance to, individuals with issues related to self-care, medication management, task completion, reduction/elimination of maladaptive behaviors, problem-solving skills, money management, pre-vocational skills, and the skills that are needed to maintain a household.

Structured-Day Program

Structured-Day Program services are provided in an outpatient congregate setting or in the community. These services are designed to improve or maintain the individual’s skills and ability to live as independently as possible in the community. Individuals may choose to engage in groups that emphasize basic work skills, problem-solving, communication, computer operation, cooking, and a wide range of other activities that work on promoting or sustaining their ability to be independent within their community.

Residential Services

Residential services are currently provided to those individuals who have sustained a traumatic brain injury and have a variety of insurances, such as Worker’s compensation, no-fault insurance, or private insurance. The residences are located close to community services and to our Structured-Day Program. These services include support with activities of daily living, community integration, homemaker or housekeeping services, and other personal-care tasks. Also, these services provide safety and oversight of those individuals who require such assistance. The residential environment enables the residents to learn at their own pace and to gain skills that facilitate greater personal independence and self-esteem.

Licensed Home Care Provided by Personal Care Aides

Our Registered Nurse provides an assessment in the development of a care plan that is centered on the needs of the client. A Personal Care Aide not only performs basic personal-care tasks but also provides support with the activities of daily living to help individuals recover in the comfort of their own home:

Prepares and serves meals to fit the nutritional needs of the client
Maintains general cleanliness of the home: launders clothing and bed linens; dusts; vacuums; washes dishes; thoroughly cleans the kitchen and bathroom
Ensures that all needed supplies are readily available for client and program use
Supports and helps clients with banking, medical appointments, and shopping
Provides assistance with self-administration of medication

Eligibility criteria

Services are provided through the Home and Community-Based Medicaid Traumatic Brain Injury (HCBS/TBI) waiver and are regulated by the New York State Department of Health. The program also provides services for individuals who have other insurances, such as New York State Insurance Fund, New York State Workman’s Compensation, and private pay.
